Who We Serve
Heart of Lynn Adult Family Home serves adults ages 18 and older who are classified by the State of Wisconsin with one or more of the following needs:
- Developmentally Disabled (DD)
- Pregnant Women Who Need Counseling (PWC)
- Irreversible Alzheimer’s/Dementia (ALZ)
- Emotionally Disturbed/Mental Illness (MH)
- Traumatic Brain Injury (TBI)
- Alcohol/Drug Dependent (AODA)
- Advanced Age (AA)
- Correctional Clients (CC)
- Terminally Ill (TI)
- Physically Disabled (PD)
